Primary Health Care
Primary health care is the backbone of Australia’s health care system. Outreach Doctors provides a warm and welcoming holistic primary health care service delivered by our medical and nursing staff and Allied Health Professionals. Our services are tailored to support diverse population groups and has a particular focus on the health and wellbeing of Aboriginal & Torres Strait Islanders Peoples. Our services support clients to address health & lifestyle conditions including but not limited to chronic conditions such as asthma, diabetes, heart disease. Outreach Doctors provides a safe space to address issues to withdraw or manage health conditions associated with legal and illegal substance misuse. Outreach Doctors can support our patients with harm minimisation or on their journey to recovery. Our aim is to support and treat our patients with individually tailored care plans that is inclusive of alternative lifestyles.
In Home Palliative Care
Our Home Palliative Care mission is to provide individuals with a peaceful dignified end in their own home.
We aim to optimise patients’ quality of life on the last part of their journey. Our supports offered are person and family-centred care.
Our multi-disciplinary team’s care is guided by an understanding that in the final stage of life, patients and their carers support & health needs are essential.
Private Nursing Care
Outreach Doctors has private nurses available for in home services. These service include, but aren’t limited to:
- Palliative care & family support
- Facilitating earlier discharges from hospital
- Assisting patients to stay in their home longer, or while they await a home care package
- Wound care & dressing changes
- Continence nurse

Community Capacity Building
Outreach Doctors supports local people to address and solve local issues, we recognise that our clients are the experts in their own lives, and we encourage them to use their strengths. We support our clients to increase their health, their skills at individual, family and community levels.
Mental Health
Outreach Doctors work with clients to recognise and understand their mental health concerns, we work with our patients and other health providers to reach optimum mental health and wellbeing. We support our clients through a variety of means using medication, counselling and where appropriate referral to other trusted supports. We understand the relationship between mental and physical health and respond to these alongside our clients by respecting their lifestyles and their preferences around treatment
Sexual Health
Sexual health incorporates physical, social and emotional wellbeing related to our sexuality. Outreach Doctors offers a safe, non-judgemental space where all aspects of sexual health and well-being can be discussed. We offer comprehensive testing and our staff are experienced in having open and respectful conversations with clients to protect sexual health, enhance intimacy and address any concerns. Outreach Doctors encourages safe sex practices.
